The information you use in this form, must match the information in your original CP-575 Letter or your replacement 147C Letter that you’ve received from your state. Once you have successfully submitted the form, keep track of your Brand ID, a copy of it will be emailed to you upon completion as well.

If you lose your original CP-575, you can request a replacement document called a 147C EIN Verification Letter.

By Phone: The only way to request a 147C letter is by calling the IRS Business & Specialty Tax Line at 1-800-829-4933.
- Be prepared to answer verification questions about your business.
- You may be able to receive the letter via fax during the call, or it can be mailed to your address of record (which takes 4-6 weeks).
